Initial Evaluation Session (60 Minutes) = $160.00
Therapy Session (45 Minutes) = $130.00
ABOUT PAYMENTS
Payment is due at the time of service in the form of either cash or a check.
At your request, a monthly statement will be provided with the information necessary for you to submit your out-of-network claim to your insurance provider for reimbursement.
ABOUT INSURANCE
I accept Aetna and Blue Cross/Blue Shield Highmark plans. Clients who have one of these plans are responsible for their co-pay or co-insurance at the time of the session and I submit the claim directly to the insurance company.
If you are not sure if I am an in-network provider with your plan, you may check by calling the Member number on the back of your card or using the Doc Finder tool on your insurance company’s website.
It is a good idea to call your insurance company before beginning therapy to check on the amount of your co-pay or co-insurance, to see if you have a deductible that must be met, and to ask if there is a maximum number of sessions allowed with your plan.
If I am not an in-network provider with your plan, I am happy to provide you with a statement that you may submit to your insurance company for reimbursement.
